July 2022 & this problem still exists. After a few hours of googling and trying all kinds of debugging, I finally figured it out. My iPhone always showed my wife as having notifications silenced. What fixed it was in her phone, go to Focus >> Do Not Disturb > Settings.
My phone tells others that I have notifications silenced, when I do not. I do not have any focus settings on, Do Not Disturb is off.
