I contacted Starz directly and they told me to do everything that I'd already done, then apologize that it wasn't working and told me to contact Roku directly. Instead, I went through all of my apps and discovered that I have a "Starz on Roku" app and the official "Starz" app. I deleted both apps, then reinstalled only the official Starz app.
