I find that games that are more gameplay-immersive (aka Boneworks and bonelab) don't require high quality graphics. But the more you step away from gameplay realism, the more graphics begin to matter. I feel like it's just a non-intrusive way of asking for a rough body size for people that don't really care. As a fat bloke I wouldn't personally care if the options were thin, medium, me, but I imagine some people would, plus it could be a bit weird to come up with body type names for XXL or higher, so shirt size seems like a good compromise to me.
